Everyone working with athletes at the Canadian Elite Softball Camp (August 4–6, 2026, Georgetown, ON) completes this once before camp. It takes about 5 minutes. Our athletes are minors — thank you for helping keep them safe.

I have never been convicted of, charged with, cautioned for, or investigated for any offence involving a minor, child sexual-abuse material, sexual misconduct, assault, or violence.
I am not now, and have never been, listed on any sex-offender or child-abuse registry in any jurisdiction.
I have never been the subject of a complaint, disciplinary proceeding, suspension, dismissal, or resignation in lieu of dismissal arising from my conduct toward a minor or an athlete.
I am not subject to any court order, restraining order, bail condition, or probation term that limits my contact with minors.
No allegation, investigation, or proceeding of the kinds above is currently pending or, to my knowledge, contemplated against me.
I will immediately notify the Camp Director in writing if any statement above changes before or during the camp.
I understand that providing false or incomplete information is grounds for immediate removal without compensation, and may be reported to the authorities.

Code of conduct

Rule of Two. I will not be alone and out of sight with a single athlete; interactions happen in an observable, interruptible setting with another screened adult present or in view — except in a genuine emergency.
Communication. I will not contact athletes privately (text, DM, social media, email, phone). All communication goes through parents/guardians or organizers.
Physical contact. Limited to what is necessary for instruction or safety, appropriate to the setting, and in view of others — never for personal gratification.
Boundaries. No romantic or sexual conduct with any athlete, ever. No providing or using alcohol, cannabis, tobacco, or unlawful drugs at camp; no sharing transportation or accommodation alone with an athlete.
Respect & inclusion. No harassment, bullying, hazing, discrimination, or demeaning language. I treat every athlete with respect.
Photography & media. Only official, organizer-approved photography. No personal photos/video of athletes; no posting athlete images to personal accounts.
Duty to report. If I suspect a child is being abused or needs protection, I will report immediately to the Camp Director and, as required by Ontario's Child, Youth and Family Services Act, to a Children's Aid Society. This overrides any confidentiality.
Health & safety. I will follow the camp's emergency procedures and Ontario's Rowan's Law concussion protocol — removing an athlete from play when a concussion is suspected and returning them only per protocol.
Consequences. Any breach may result in immediate removal and, where warranted, referral to authorities and governing bodies.
